A Clinical Study of 175 Cases of Anal Fistula



Abstract
This is a report of 175 cases of anal fistula which were treated at the general surgery in Hanyang University Hospital from May 1972 to July 1983.
The results were as follows;
1) In the 953 cases of anorectal disorders except congenital malformations, the anal fistula cases occupied 18.4% of all.
2) The sex ratio was male preponderance as 4: 1 and the most prevalent age group was 2nd decade as 41.1% of all.
3) The most common etiologic factor of all fistula was perianal abscess as 47.4% of all and tuberculous origin was accounted for 4.5%.
4) The onset before admission was within one year in 107 cases (61. 1%).
5) Main subjective symptom of the cases were pus discharge on anus, pain, bleeding in orders.
6) Chest x-ray revealed normal in 152 patients (66.9%), active pulmonary tuberculosis in 13 patients (7.4%o) and inactive pulmonary tuberculosis in 10 patients (5.7%). 7) Caudal anesthesia was done over half cases.
8) Intersphincteric type was most frequent as 60.0% of all in anatomical type of anal fistula.
9) On physical examination, single external opening was 152 patients (86.9%) and single internal opening was 156 patients (89. 2%) and internal opening was not found in 10 patients.
10) The most frequent site of external opening at the lithotomy position was 1, 6, 7, 11 o¢¥clock in direction and was 56% of all external openings.
11) Fistulectomy was applied to the most patients (69. 7%).
12) The most common histopathological finding of anal fistula was chronic non-specific inflammation (77 cases).
13) Postoperative complication was recurrence (6.3%), and bleeding and abscess.
14) The average of admission day was 10 day.
